Financial Information
On your behalf, Memorial Medical Center will bill your primary and secondary insurance plans if
you provide us with this information at the time of registration.
You are ultimately responsible for payment of all charges if your insurance does not pay.
If you are not insured, payment in full is expected within 30 days from the billing date
If you are unable to pay your bill within 30 days, contact a Patient Financial Specialist at
715-685-5500 to set up a payment plan or to discuss other arrangements.
Billing Policy
Please be aware that each time you receive services at Memorial Medical Center, a separate account is
created. If you are receiving monthly ongoing services, a monthly account is created. It is possible to
have several accounts open at the same time for which you will receive separate statements. Your
statements may reflect accounts billed to your health plan that are not yet resolved, or balances that
you owe.
Billing for Separate Services
Depending on the services provided, you may receive separate bills from other health care
professionals who provided services to you at Memorial Medical Center.
Charges for anesthesiologists, radiologists, pathologists, surgeons and attending physicians are
billed separately.
These separate services may not be contracted through your HMO or PPO. For your convenience

Paying Your Bill
Memorial Medical Center will accept:

- Money Orders

Payments can be made in person at the cashier's office located at the Main Entrance of the
hospital.
To pay by credit card, call 715-685-5500.
By mail, please include your patient account number on the front of the check, insert the stub from
your statement and send the payment to:

Financial Assistance
At some time, almost every family is touched with an unexpected need for medical care. If you do not
have insurance and are unable to pay for your medical services, you may qualify for financial assistance
through Memorial Medical Center's Community Care program. If you need help in meeting your hospital
bill obligations, please contact our Business Office at 715.685.5500.
In the Meantime...
Financial counseling is available in person Monday through Friday at the cashier's office located at the
Main Entrance of the hospital from 7:00 am to 5:00 pm. You may simply drop in or call to speak to one
of our friendly Patient Financial Specialists at 715-685-5500. You may also fax us at
